A planned pathway

Our Layout
Each floor has an individual model of care, created for the needs of the service-users. Our Ground Floor, known as Hornbeam accommodates adults between 18 and 65 who have a mental health diagnosis or require mental health support. The increased staffing levels on this floor allow us to identify triggers, therefore working with service-users to reduce their behavioural challenges and supporting them to access the local community where previously they may have been considered high-risk.

The first floor, known as Silver has been designed specifically for the needs of residents over the age of 65 living with a dementia diagnosis or Mental Health with a dementia diagnosis. Our homely environment, facilities and model of care allows service users to retain as much independence as possible whilst keeping them safe and well cared for.
Acer, our 2nd floor accommodates those with Complex Medical Health conditions such as Motor Neurones and Huntington’s, people with Tracheostomy Care, ABI, NIV, BIPAP and CPAP.
Throughout the home, our technology systems, which include Nourish, V care, and care quality Pharmacy service, have been set up to be integrated, allowing managers to have full oversight of residents at all times.
