Located in the heart of Wolverhampton, Maplebrook Care Home is a modern, purpose-built service providing high-level care for adults with complex needs. We work closely with healthcare and social care professionals to ensure timely, safe and appropriate placements across a range of specialist care pathways.

Tailored Support and an Expert Team

Maplebrook Care Home offers 24-hour nursing care for adults with complex physical health conditions, mental health needs, and progressive neurological illnesses. We accept both short- and long-term placements and can support step-downs from hospital or ongoing community placements. Our model is underpinned by personalised care planning, clinical oversight, and a strong focus on outcomes, ensuring that each individual receives care tailored to their diagnosis, presentation and goals. Our team includes experienced nurses, support staff and management who work in collaboration with MDTs to deliver safe, consistent and responsive care.

Dementia Care

Our dementia care service supports adults over 65 with a confirmed diagnosis of dementia, including those with additional mental health or physical care needs. The environment on our dedicated dementia floor, Silver, has been adapted to reduce distress and promote familiarity, with clear signage, calm décor and a structured daily routine. Staff are trained in dementia-specific care, including de-escalation, communication strategies and management of responsive behaviours.

We focus on maintaining existing skills and promoting engagement through life story work, structured activities and a calm, consistent approach. Individual care plans are created with input from families and external professionals, ensuring needs are met in a holistic, person-centred way. We also work closely with GPs and local memory services to ensure continuity of care.

Complex Nursing Care

Maplebrook provides 24-hour nursing care for adults with a range of complex conditions including Parkinson’s disease, Huntington’s disease, acquired brain injury, Motor Neurone Disease, spinal injuries, and respiratory support needs (e.g. tracheostomy, CPAP, BiPAP and NIV). Our Acer floor is fully equipped to manage clinical interventions and offers an appropriate environment for individuals with significant medical or mobility needs.

Each resident has a comprehensive care plan that includes risk assessments, manual handling guidance, nutrition, respiratory care and medication management. We use integrated care systems, including Nourish, VCare, and a digital pharmacy service, giving managers and MDTs full clinical oversight. Our focus is on safety, dignity and supporting quality of life in a setting that feels stable and well-structured.

Mental Health Care

Our mental health service, based on the Hornbeam floor, provides specialist support for adults aged 18 to 65 living with complex and enduring mental health conditions, including schizophrenia, bipolar disorder, and personality disorders. Residents may have a history of high-risk behaviour or difficulty maintaining placements in other settings. We provide a high staff-to-resident ratio and a therapeutic, structured environment.

Our approach is based on positive behaviour support and risk management, with a focus on community integration where appropriate. The team has experience in identifying and responding to early signs of behavioural escalation and works in close partnership with mental health services, social workers and crisis teams. Personalised care pathways are created for each resident, including discharge planning where clinically appropriate.

Palliative and End-of-Life Care

We provide nurse-led palliative and end-of-life care for individuals with life-limiting conditions. Our service includes symptom management, pain control and full emotional and practical support for both residents and families. The aim is to ensure comfort and dignity throughout the final stages of life, in a peaceful and well-supported environment.

Care is delivered in partnership with local palliative care teams, GPs and other healthcare professionals, ensuring a consistent and multi-disciplinary approach. Open visiting, private family spaces and staff experienced in end-of-life care contribute to the calm and supportive atmosphere we maintain during this stage.

Maplebrook Care Home provides a flexible and responsive approach to both long- and short-term placements for adults with complex health and social care needs. Our team is experienced in supporting individuals with dementia, enduring mental health conditions, neurological diagnoses, physical disabilities, and high-level nursing needs.

We work closely with referring professionals, individuals and families to ensure that every placement is clinically appropriate and aligned with the resident’s needs and goals. Our Clinical Assessors carry out timely assessments following referral, enabling us to support smooth, well-planned admissions. From initial enquiry through to transition, we prioritise continuity of care, clear communication and a safe, dignified start to life at Maplebrook.

What information is needed to make a referral?

We ask that referrers provide a recent care needs assessment, risk assessment, relevant clinical history, current medication list, and any behavioural support plans if applicable. This helps us ensure an accurate and timely assessment of the individual’s needs.
